Data Protection and Confidentiality

Your personal information has been provided to Quality Health so that they can carry out the Cancer Quality of Life Survey on behalf of NHS England. There are strict data security arrangements in place to keep your information safe and your answers will be dealt with in confidence. The survey will help us to improve the way we support people to live as long and as well as possible.

Under data protection laws, there are certain circumstances where personal information can be shared without asking people for their permission, for example where this is necessary for a task carried out in the public interest or for official functions. This survey is covered by those circumstances as it will help the NHS improve the way we support people to live as long and as well as possible. Further information about this can be found in the Privacy Notice shared by the National Disease Registration Service, which is part of NHS England.

Quality Health is a specialist survey company and is fully compliant with Data Protection Laws including the General Data Protection Regulation and the Data Protection Act 2018 (ICO registration number: Z6385547); and the Market Research Society’s Code of Conduct.

Quality Health takes its data protection responsibilities very seriously and have robust controls in place to ensure your information is protected at all times from loss, theft or misuse. Strict measures are in place to ensure personal information is securely stored and is only seen by employees directly involved in the survey who have received special training on confidentiality and data protection. All systems and processes are governed by ISO27001:2013 and ISO9001:2015, which are the International Standards for Information Security Management and Quality Management.

Yes. You have the right to request the following:

  • confirmation that your data is being processed and
  • access to your personal data.

Right to access requests will be responded to without delay and at the latest within one month of receipt. Yes. You have the right to have inaccurate personal data corrected, or completed if it is incomplete. Requests for corrections will be responded to within one calendar month. Alternatively, you can write to FREEPOST Quality Health (you do not need a stamp).

Yes. You have the right to request Quality Health to delete your personal data at any time up to the point at which they send your answers back to the National Disease Registration Service, which is part of NHS England.

If you choose to complete and return the questionnaire you agree to your personal data being processed for the purposes required for the survey. If you change your mind after completing the survey, you have the right to restrict the processing of your answers at any time, up to the point at which Quality Health send your answers back to the National Disease Registration Service, which is part of NHS England. This number starts with the letter P and is followed by 8 numbers e.g. P12345678.

You do not need to give us a reason and it will not affect the care you receive from the NHS in any way. NHS staff will never be made aware of your decision.

Quality Health will only hold your data in a way that can identify you for as long as is needed to invite you to complete the survey and monitor completion. Quality Health will remove your personal, identifying data from their systems six months after the survey invitation letter has been sent out.
